with Narrative Report $ 1,500.00 to $1,950.00 *
(Preferred examination for litigation, Fitness For Duty Evaluation, LTD. Includes dual clinical interviews and psychological testing).

Simple Psychiatric Examination with Narrative Report (Psychiatrist alone, no psychological testing, suitable for simple LTD examinations and non litigated cases) :$ 750.00 to $950.00 *

730 Court Ordered Child Custody Evaluation. From $.3,500.00 (2 Adults, 1 Child) A non-refundable, true case retainer, in advance, of Retainer, services to be billed against at a rate of $175.00 office, $250.00 non-office, and other fees services (base: 2 adults, 1 child) as may be required, Deposition & Testimony extra.
Deposition Rates:
Psychiatrists: $500.00 per hour.
(Two hours minimum, paid ONE WEEK in advance of deposition, balance, if any, at completion)
Dr. Spatz: $ 400.00 per hour. (Two hours minimum, paid ONE WEEK in advance of deposition, balance, if any, at completion)
Expert Witness Fee - Dr. Spatz(Appearance): $ 2,000.00 (half day)
(50% in advance, balance at start of testimony): $ 3,250.00 (full day)
Psychiatrists Expert Witness Fee @ $500.00 per hour in Office-Appearances higher. (Two hours minimum, paid ONE WEEK in advance of deposition/court appearance)
Other Services:
Document and Record Reviews/Analysis: Starting at $250 per hour.
Psychiatric Treatment: $225.00 per session (1 hour).
Medication evaluation and management: variable rate
Psychotherapy: Rates variable from $98.40 per treatment session.
Hypnotherapy. . . .variable rate
Critical Incident Debriefing:
Emergency: from $300.00 per hour
On Site: from $350.00 per hour + travel time
Office: from $200.00 per session
Initial Telephone Case Consultation: No Charge
Telephone Consultation-Ongoing: from $150.00 per hour
